중환자의 혈당 조절: 최신 업데이트

Abstract

Proper glucose management in hospitalized patients can improve clinical outcomes. In particular, intensive care unit (ICU) patients are known to have significantly higher rates of mortality from changes in blood glucose due to severe comorbidities. Improving glucose control in ICU patients, therefore, can improve mortality and prognosis. Several studies related to the management of blood glucose in the ICU patients have been conducted. Intensive glucose management of surgical ICU patients has been successful. However, studies on medical ICU patients did not demonstrate positive effects of strict glycemic control. There is no independent glucose management goal for neurological ICU patients. However, maintenance of the usual glucose control target of 140-180 mg/dL is recommended for ICU patients. Intravenous insulin infusion is essential for glucose control in ICU patients not consuming a regular diet, and caution should be exercised to prevent hypoglycemia.

서 론

당뇨병 환자는 당뇨병을 동반하지 않은 환자에 비하여 응급실 내원 혹은 입원 빈도가 많고, 입원 기간이 길며, 다양한 검사 및 처치를 수행하는 경향을 보인다[1,2]. 또한 만성 합병증, 감염, 고위험 임신 등 일반 인구보다 좀 더 불량한 임상 상황에 노출되는 빈도가 높다[3].
병원에 입원한 당뇨병 환자 중 특히 중환자에서 안정된 혈당 조절은 환자의 예후 향상에 중요한 영향을 미친다[4,5]. 적극적 혈당 조절을 통하여 환자의 임상 경과를 호전시키고, 관련된 의료 비용을 경감시킬 수 있다. 본 고찰에서는 중환자 혈당 조절의 중요성에 대한 최근 연구 결과 및 진료지침의 내용을 간단히 소개하려 한다.

본 론

1. 입원 환자에서 당뇨병과 고혈당

급성 질환(acute illness)은 체내 에피네프린, 노르에피네프린 등 스트레스 호르몬의 수치를 높여 혈당 상승의 원인으로 작용한다[4,6]. 급성 질환의 치료에 흔히 사용되는 스테로이드, 장관영양(enteral feeding), 완전비경구영양(total parenteral nutrition) 등의 처치 역시 혈당 상승의 원인이 된다[4,7]. 혈당 상승은 체내 면역력 저하, 창상 회복 지연, 산화스트레스 증가, 혈관 내피세포 기능부전, 염증 반응 증가 등의 기전으로 급성기 임상 경과를 악화시킨다[4,6].
입원 환자에서 불충분한 혈당 조절은 환자의 경과와 예후에 악영향을 미치는 중요 요인이다. 한 연구 결과, 입원 24시간내 고혈당은 환자의 사망률을 예측할 수 있는 중요한 지표로 확인되었다[8]. 이미 당뇨병 병력을 알고 있던 환자보다 입원 중 새로 확인된 고혈당 환자에서 더욱 부정적 임상 경과를 보였다. 지역사회 기반의 한 연구 결과, 종합병원 입원 환자의 38%에서 고혈당을 동반하였다[9]. 전체 고혈당 환자의 약 30%는 기존에 고혈당 관련 병력을 인지하지 못하였는데, 이들의 비중 환자실 사망률과 중환자실 사망률이 유의하게 높았다[9]. 하지만 실제 급성기 입원 환자는 입원의 직접 원인이 당뇨병과 무관하므로, 이들에게 엄격한 혈당 조절의 필요성은 종종 간과된다.

2. 중환자에서 엄격한 혈당 조절의 근거

중환자실 환자의 혈당 조절 불안정성은 일반 입원 환자보다 임상 경과에 더욱 유의한 영향을 미친다. 한 후향적 연구 결과, 평균 혈당치가 높을수록 중환자의 사망률이 증가하였다[5]. 중환자의 혈당을 정상에 가깝게 조절한다면 환자들의 사망률, 이환율 등 임상 경과 개선에 도움이 될 수 있다.
관상동맥우회술 시행 환자를 대상으로 수행된 포틀랜드 당뇨병 프로젝트(Portland diabetes project) 연구 결과, 혈당을 200 mg/dL 이상으로 조절한 사람과 그 미만으로 조절한 사람들의 사망률은 각각 5.0%, 1.8%로 유의한 차이를 보였다[10]. 이 코호트에서 혈당을 좀 더 안정적으로 조절하기 위한 연속 인슐린 정주(continuous intravenous insulin, CII)의 효과를 슬라이딩 스케일 인슐린 피하주사(sliding scale guided intermittent subcutaneous insulin, SQI)와 비교하였다. 그 결과, 대상자의 심부 흉골 감염(deep sternal wound infection, DSWI) 빈도가 0.8%, 2.0%로 유의한 차이를 보였다[10]. 실제 이 코호트의 장기 관찰 결과 혈당 조절 방식을 SQI에서 CII로 변경 후 대상자들의 DSWI와 사망률 등 임상 경과에 지속적인 개선을 확인하였다[11].
DIGAMI 연구는 급성 심근경색으로 심장동맥집중치료실에 내원한 제2형 당뇨병 환자 620명을 대상으로 시행되었다[12]. 대상자는 연속 포도당-인슐린 정주(continuous glucose-insulin infusion)군과 통상 치료군으로 나누어 중재가 이루어졌다. 그 결과, 입원 첫 24시간 내 혈당은 연속 포도당-인슐린 정주군에서 277.2±73.8 mg/dL에서 172.8±59.4 mg/dL로, 통상 치료군에서 282.6±75.6 mg/dL에서 210.6±73.8 mg/dL로 변화하였으며, 양 군 간 유의한 차이를 보였다[12]. 이들의 1년 후 사망률은 연속 포도당-인슐린 정주군에서 18.6%, 통상 치료군에서 26.1%로 유의한 차이를 보였다[12].
외과 중환자실 입원자 1,548명을 대상으로 시행된 대규모 무작위 대조 중재 연구 결과, 혈당 80-110 mg/dL를 목표로 엄격하게 조절한 환자들의 12개월 사망률은 4.6%, 통상 치료군 8.0%로 유의한 차이를 보였다[13]. 80-110 mg/dL의 엄격한 혈당 조절은 사망률을 감소시켰을 뿐 아니라, 혈류 감염을 46%, 급성신부전 및 신대체 요법 시행을 41%, 수혈을 50%, 중환자 다발신경병증(critical-illness polyneuropathy)을 41% 낮췄다[13]. 하지만 이후 같은 연구자가 내과계 중환자를 대상으로 시행한 대규모 무작위 대조 중재 연구 결과 80-110 mg/dL의 엄격한 혈당 조절은 원내 사망률을 개선시키지 못하였다[14].
중환자 혈당 조절 목표에 대하여 최근까지 시행된, 확인하기 위한 가장 큰 규모의 무작위 대조 연구는 Normoglycemia in Intensive Care Evaluation–Survival Using Glucose Algorithm Regulation (NICE-SUGAR) 연구이다[15]. 이 연구는 6,104명의 환자들이 참여하였으며 혈당 조절 목표 81-108 mg/dL의 집중 치료군, 혈당 조절 목표 180 mg/dL 미만의 통상 치료군으로 나누어 이들의 임상 경과를 확인하였다[15]. 그 결과, 대상자의 90일 사망률은 통상 치료군에 비하여 집중 치료군에서 유의하게 높았다. 치료의 효과는 외과계 중환자실과 내과계 중환자실 간 유의한 차이를 보이지 않았다[15]. 집중 치료군의 6.8%에서 40 mg/dL 이하의 심한 저혈당이 발생한 반면, 통상 치료군의 저혈당 발생은 0.5%로 차이가 있었다[15]. 양 군 간 재원 기간, 기계환기 혹은 신대체 요법 유지 기간에 유의한 차이는 없었다[15].
신경계 중환자실 환자만을 대상으로 시행된 무작위 대조 중재 연구는 드물다. 급성 허혈성 뇌졸중 환자에서 신속히 혈당을 낮추고, 이를 유지하기 위한 연속 포도당-칼륨-인슐린 정주군과 식염수를 주사하며 혈당 306 mg/dL 이상에만 피하 인슐린 주사를 시행한 대조군을 비교한 The UK Glucose Insulin in Stroke Trial 연구 결과 대상자의 90일 사망률에 유의한 차이는 없었다[16].
무작위 대조 중재 연구 외 외상 뇌손상, 허혈성 혹은 출혈성 뇌졸중, 무산소뇌증(anoxic encephalopathy), 척수 손상 및 중추신경계 감염 등 신경계 중환자실에 내원한 환자를 대상으로 진행된 메타분석 결과를 확인할 수 있다[17]. 이 연구 결과, 신경계 중환자실 환자에서 70-140 mg/dL의 엄격한 혈당 조절이 사망률 개선에 미치는 영향은 분명하지 않았다. 하지만 엄격한 혈당 조절을 시행한 환자에서 불량한 신경학적 임상 경과가 유의하게 적었다[17].
NICE-SUGAR 연구에서 외상 뇌손상 환자들을 추려 추가로 분석한 연구 결과에서도 집중 치료군과 대조군 간 임상 경과에 유의한 차이는 확인되지 않았다[18]. 최근 신경계 중환자에 대한 특화된 치료가 환자의 경과 개선에 도움이 될 수 있다는 근거가 있다[19]. 따라서, 신경계 중환자실 환자의 혈당 조절 목표에 대한 좀 더 많은 근거가 마련될 필요가 있다. 현재 약 1,400명의 급성 허혈성 뇌경색 환자를 대상으로 80-179 mg/dL의 엄격한 혈당 조절이 임상 경과 개선에 영향을 미치는지 확인하기 위한 무작위 대조 연구가 진행되고 있다[20]. 향후 신경계 중환자실 환자의 혈당 조절 목표에 대한 양질의 근거가 마련될 것으로 예상된다.

3. 실제 임상지침

실제 병원 중환자실에서 혈당을 관리하는 주 목적은 혈당 변화로 인한 대사적 이상을 최소화하고 부작용 발생을 예방하며, 신속히 일반 입원실 혹은 외래 진료로 전환하는데 있다. 현재까지의 주요 연구 결과를 요약하면, 상기 목적을 달성하기 위하여 가장 적합한 중환자실 입원 환자의 혈당 조절 목표는 180 mg/dL 미만이다. 국내외 주요 지침에서 140-180 mg/dL 유지를 원칙으로 하며, 110 mg/dL 이하의 엄격한 혈당 조절은 권장하지 않는다(Table 1) [21-27].
중환자실의 혈당 조절은 인슐린 정주를 원칙으로 한다[22]. 안정된 혈당 조절을 위하여 사용하는 몇 가지 프로토콜이 있는데, 그 기본은 거의 같다[13,28,29]. 혈당을 반복적으로 측정하고, 이에 따라 수액에 희석된 인슐린 주입량을 변화시켜 조절한다. 물론, 혈당 이외에도 전해질 등 인슐린 치료에 따라 변화할 수 있는 임상 지표를 함께 안정시켜야 한다. 필자는 Joint British Diabetes Societies for Inpatient Care의 프로토콜을 실무에 자주 활용한다(Table 2) [25].
중환자의 회복기에는 피하 인슐린 사용을 정당화할 수 있다. 일반적으로 0.5-0.7 단위/kg의 인슐린을 사용하며, 기저량과 식전 인슐린의 비율은 통상 50:50으로 시작한다[30]. 혈당 측정 결과에 따라 인슐린을 피하 주사하는 ‘슬라이딩 스케일’ 인슐린 주사는 혈당 변동성이 크고, 지나친 고혈당 혹은 저혈당 발생을 초래할 수 있어 권장하지 않는다[31]. 실제 Randomized Study of Basal-Bolus Insulin Therapy in the Inpatient Management of Patients With Type 2 Diabetes 연구에서 슬라이딩 스케일과 기저-식전 인슐린 요법 간 임상 경과를 비교한 결과, 기저-식전 인슐린 요법의 혈당 강하 효과가 유의하게 좋았다[32].
최근 개발된 다양한 약제는 저혈당 위험과 약물 상호작용의 위험이 적어 그 적응증이 상대적으로 넓다[33]. 하지만 중환자 치료에 대한 당뇨병 신약의 효과는 아직 입증되지 않았다. 또한, 중환자 치료에 연속 혈당측정기, 인공췌장 등 최신 장비를 이용하여 엄격한 혈당 조절을 유지하면서도 부작용을 최소화하는 치료 전략을 고려할 수 있다[34,35]. 향후 중환자 혈당 관리에 이러한 최신 혈당 치료 기법과 관련된 다양한 근거가 마련될 필요가 있다.

결 론

중환자 치료에 있어 적절한 혈당 조절을 유지하여 환자의 경과와 예후 개선이 가능하다. 중환자실 혈당 조절에 대한 초기 연구 결과, 엄격한 혈당 조절은 환자의 임상 경과 호전에 유의한 효과가 있었다. 하지만 좀 더 엄격한 혈당 조절 목표에 대한 후속 연구 결과 지나치게 엄격한 혈당 조절은 오히려 환자의 임상 경과와 예후에 부정적 영향을 초래하였다.
현재 대부분의 지침에서 중환자실 환자들의 혈당 조절 목표는 140-180 mg/dL를 권장하며, 인슐린의 정주 치료를 원칙으로 한다. 신경계 중환자실 입원 환자에 대한 독립적 혈당 조절 목표는 아직 존재하지 않으며, 일반 중환자실 환자의 혈당 조절 목표를 준용한다.
최근 다양한 약제와 장비가 개발되고 있어 좀 더 엄격한 혈당 조절을 유지하면서도 부작용을 최소화하는 치료 전략을 제안할 수 있다. 향후, 이러한 관점에서 좀 더 많은 연구가 필요하다.

50 units of insulin in 49.5 mL of 0.9% NaCl solution yields a concentration of 1 unit/mL.

If the patient usually takes basal insulin, continue to use basal insulin with intravenous infusion.
