뇌실 외 배액관 관련 뇌실염의 예방과 관리

Abstract

Ventriculitis after extraventricular drain (EVD) placement causes significant morbidity and mortality. This review is focused on the prevention and management of EVD-related ventriculitis. To prevent EVD-related ventriculitis, standardized EVD care, including sterile handling and strict infection control performances, and repeated education for healthcare workers are essential. Routine change of EVD is controversial. It can be considered when EVD is needed for more than 7 days. Perioperative antibiotic prophylaxis beyond 24 hours is not recommended, except for cerebrospinal fluid (CSF) leakage. Antibiotic-impregnated EVD catheter seems to reduce the incidence of EVD-related ventriculitis. CSF sampling for early detection of EVD-related ventriculitis can be considered 4 to 5 days after EVD insertion. Linezolid can be used for treating ventriculitis caused by multidrug-resistant Gram-positive organisms. Treatment options for extensively drug-resistant or pandrug resistant Gram-negative bacilli are scarce. Intraventricular colistin or aminoglycoside can be considered for those conditions.

서 론

뇌실외배액술(extraventricular drain, EVD)은 두개내압을 측정하고 증가된 압력을 감소시키는 역할을 하며, 이는 외상뇌손상(traumatic brain injury)이나 출혈성 뇌졸중(hemorrhagic stroke) 환자 치료에도 유용한 수단이다[1]. 하지만 EVD 사용은 동시에 감염의 위험에 노출되게 되는데 주된 감염 합병증은 뇌실염(ventriculitis)이다. EVD 관련 뇌실염은 EVD를 하는 환자의 4-23% 정도에서 발생한다[2]. 지금까지 알려진 EVD 관련 뇌실염 발생의 위험인자로는 뇌실내출혈, 뇌척수액 누출(leak), EVD 관 세척행위, EVD 거치 기간이 긴 경우 등이 있다[3-8]. 본 종설에서는 EVD와 관련 뇌실염의 예방과 진단∙치료에서의 몇 가지 중요한 문제들에 대해 가급적 최근 자료들을 중심으로 정리해 보고자 한다.

본 론

정기적인 EVD 교체는 뇌실염 발생 위험을 줄일 수 있을까?

정기적인 EVD 교체의 효용성에 대한 논란은 오래되었지만 아직까지 결론이 나지 않고 있는 문제이다. 이에 대한 대표적인 연구들을 소개하고자 한다. 1970년 말에서 1980년대 초에 미국에서 전향적으로 수행된 EVD 관련 감염에 관한 한 연구에서 EVD 관련 뇌실염의 위험이 EVD 삽입 5일 이후에 급증하는 것을 발견한 연구자들은 5일 이상 EVD 유지가 필요한 경우 EVD를 제거하고 다른 부위에 삽입할 것을 제안하였다[3]. 1990년대 말에 이르러 홍콩의 한 병원에서 5일마다 EVD를 제거하는 무작위 배정 연구를 하였는데, 감염률은 정기적 교체군과 대조군에서 유의한 차이를 보이지 않았다(7.8% vs. 3.8%, P=0.50)[9]. 이후에 호주의 한 병원에서 수행된 후향연구를 통해 연구자들은 EVD 유지기간보다는 EVD 삽입 횟수가 2번 이상인 경우 EVD 관련 감염이 증가한다고 주장하였다[10]. 즉, 정기적으로 EVD를 교체하게 되면 이와 관련하여 오히려 감염의 위험이 증가할 수 있다는 주장을 하였다. 최근 그리스의 연구자들은 중환자실 환자들을 대상으로 EVD 관련 감염을 줄이는 여러 가지 방법을 동시에 시행하는 이른바 bundle의 일부로 7일마다 정기적으로 EVD를 교체하는 것을 포함하였다[6]. Bundle 도입 전후를 비교할 때 EVD 관련 뇌실염은 57% 감소하였다. Bundle에 포함된 다른 항목은 중환자실 직원들에 대한 감염관리 교육, 철저한 무균술을 포함한 철저한 EVD 관리, 뇌척수액 검체 채취를 최소화하는 것이었다. 이 연구결과는 몇 가지 면에서 주목할 만 하다. 한 가지는 기존의 무작위 연구에서 효과적이지 못했던 5일마다의 교체를 7일로 늘려서 시도했다는 점이고, 또 하나는 EVD 교체와 관련하여 발생할 수 있는 감염의 위험을 직원 교육과 철저한 무균술 준수, 검체 채취 최소화를 통해 낮춤으로써 결과적으로 뇌실염 발생 위험을 낮추었다는 점이다. 여기에서 시사하는 바는 EVD 교체 여부에만 관심을 두고 다른 요인들을 등한시한다면 최종 목적인 뇌실염 발생 빈도의 감소는 이루기 어려울 수 있다는 것이다. 어쩌면 EVD 교체보다 bundle 도입을 통한 노력이 감염률을 낮추었을 수도 있다. 실제로 정기적인 EVD 교체 없이도 bundle 도입을 통해 EVD 관련 감염을 감소시켰다는 보고들이 있다[11,12]. 저자 개인의 의견은 이러한 포괄적인 노력을 함께 할 수 있다면 7일 정도 간격의 정기적인 EVD 교체가 EVD 관련 뇌실염을 줄이는 데에 추가적으로 기여할 수도 있겠다는 것이다. EVD 유지 기간이 7일 이상 길어질수록 뇌실염 위험은 더욱 커질 것이기 때문이다[2,13]. EVD를 포함한 기구관련 감염(device-related infection)에는 세균이 분비하는 물질(extracellular polymer)이 기구 표면에 균막(biofilm)을 형성하는 것이 중요한 역할을 함이 잘 알려져 있다[14]. 최근 스페인의 연구자들은 32명의 연속 환자에서 제거된 EVD에서 도관 표면의 biofilm 존재여부를 측정하였는데, 7일 이상 사용 후 제거된 EVD의 88%에서 biofilm이 존재함으로 보고하였다[15]. 정기적인 EVD 교체는 교체 수술과 관련된 출혈 같은 위험을 감수해야 하는 단점이 있다. 따라서, 개개의 환자별로 심한 출혈경향 등 EVD 교체와 관련된 위험도에 대한 고려가 필요하겠다. EVD를 교체할 때 감염이 증명되었거나 임상적으로 강력히 의심되는 경우라면 같은 부위에 다시 새로운 도관을 삽입하는 것은 재감염의 위험이 크므로 금기이다. 감염이 없는 경우도 기존에 형성된 길(tract)을 통해 뇌척수액 누출 위험이 증가할 가능성이 있고 이는 감염 위험을 증가시키므로 감염위험 측면에서 보면 처음과 다른 곳에 새로운 EVD 관을 삽입하는 것이 바람직할 것 같다. 이에 대한 비교 연구는 아직까지 없다.

EVD를 가진 환자에서 뇌실염 조기 진단을 위해 뇌실염의 증상이나 징후가 없어도 매일 뇌척수액 검사를 할 것인가?

적어도 EVD 삽입 후 초기 3-4일 동안은 뇌실염 진단을 위해 매일 뇌척수액 검사를 하는 것이 권장되지 않는다[16,17]. 이 시기에는 감염 위험이 높지 않고, 뇌척수액 검사를 위한 EVD 조작이 오히려 감염의 위험을 증가시킬 수 있기 때문이다[18]. 하지만, EVD 관련 뇌실염이 발생하더라도 의료진이 인지할 수 있는 증상과 징후가 뚜렷하지 않은 경우가 많고[19], 뇌실염의 조기 진단으로 적절한 조기 치료를 할 수 있다는 점에서 EVD 관련 뇌실염의 위험이 높은 상황에서는 1-2일에 한번 정도 뇌척수액을 검사를 고려할 만하다. 최근 영국에서 수행된 한 후향 연구에서는 무균술을 잘 지키면 cerebrospinal fluid (CSF) 채취를 하는 것이 감염률을 증가시키지 않는다고 보고하였다[20]. 서론에서 언급한 EVD 관련 뇌실염의 위험인자들 중에서 감염의 가장 흔하고 중요한 위험인자는 EVD 유지기간이 길어지는 것이다. EVD 삽입한 날을 기준으로 5일째 정도부터 뇌척수액 검사를 1-2일에 한번 정도 하는 것이 적당하겠다. 뇌실 내출혈이 있는 경우 감염의 위험이 증가하기는 하지만 이는 결국 EVD 유지기간에 큰 영향을 받으므로 뇌실내출혈이 있다고 해서 EVD 삽입 초기부터 매일 뇌척수액 검사를 하는 것은 바람직하지 않다. 뇌척수액 누출이 있는 경우는 그 자체로 뇌실염의 위험이 증가하므로 정기적인 뇌척수액 검사를 고려할 수도 있겠는데, 이 또한 감염의 위험을 증가시켜 결국 뇌실염 발생 위험이 배가 되므로, 앞에서 언급한 것처럼 뇌척수액 누출이 해결될 때까지 뇌실염 치료에 준해서 예방적 항균제를 유지하는 것이 바람직하겠다. 뇌척수액 검체 채취 시에 철저히 무균술을 지키도록 상세한 술기에 대한 교육을 하고 준수 여부를 꾸준히 확인하는 것이 중요하겠다. 요약하면, 뇌실염을 의심할 만한 증상이나 징후가 없다면 EVD 관련 뇌실염의 조기 진단을 위한 뇌척수액 검사는 삽입 후 5일째 정도부터 1-2일에 한번 정도 하는 것이 바람직하겠고, 뇌척수액 검체 채취 시 철저한 무균술 준수가 필요하다.

EVD 삽입 시 vancomycin을 예방적 항생제로 사용할 것인가?

미국 감염학회가 참여한 2013년 수술의 예방적 항생제 사용 지침에서는 청결한 신경외과적 수술의 경우 1세대 cephalosporin인 cefazolin 사용을 1차적으로 추천하고 있으며 β-lactam 계열의 항생제에 부작용이 있는 경우 clindamycin이나 vancomycin 사용을 추천하고 있다[21]. Cefazolin 대신 중추신경계로의 투과성이 더 좋은 cefuroxime을 사용하자는 주장도 있다. 아예 vancomycin을 선택적 예방적 항균제로 사용하자는 주장도 있는데, 이는 EVD 삽입과 관련된 감염의 원인균으로 흔한 methicillin-resistant staphylococci, propionibacterium, corynebacterium 등의 그람양성 세균을 고려하여 처음부터 vancomycin을 사용하자는 주장이다. 아직까지 cefazolin과 vancomycin을 효과를 비교한 무작위 대조군 연구는 없다. 이에 대해서는 각 기관별로 vancomycin이 보다 효과적일 지에 대한 고민이 필요한데, EVD 관련 뇌실염의 주요 원인균들이 실제로 위에서 언급한 methicillin-resistant staphylococci 같은 그람양성 세균들이고, EVD 삽입 7일 이내에 발생하는 경우가 많다면 vancomycin을 고려할 만하다. 그 외, 최근 항생제 사용력이 있거나, 앞에서 언급한 그람 양성세균들이 임상 검체에서 분리된 적이 있는 경우, 최근 수술력이 있는 경우는 vancomycin 사용도 타당하다고 하겠다.

EVD 유지 기간 내내 예방적 항생제를 사용할 것인가?

EVD 삽입 시에 사용하는 항생제의 추천되는 사용기간은 24시간 이내다[21]. 하지만, 실제로 24시간 이상 예방적 항생제를 유지하는 경우가 상당히 많은 것 같다. 전자 메일 설문을 이용한 한 연구에서는 조사에 응한 신경외과 의사의 56.3%가 EVD 유지 기간 내내 항생제를 사용한다고 응답하였다[22]. 이 문제에 대해서는 매우 제한된 연구 자료가 있다. 1990년대에 수행된 한 무작위 배정 연구에서는 EVD 삽입 시에만 ampicillin/sulbactam을 사용한 대조군과 EVD 유지 기간 내내 ampicillin/sulbactam과 aztreonam을 사용한 환자군을 비교하였는데, 후자에서 감염률이 의미 있게 낮았다(3% vs. 11%, P=0.01)[23]. 하지만, candida나 methicillin-resistant S. aureus 같은 내성균에 의한 감염이 대조군에 비해 더 많았다. 이 연구는 무작위 배정 연구이긴 하지만, 같은 종류의 항생제를 사용하지 않았기에 제대로 된 비교 연구라고 하기는 힘들다. 이후에 미국의 한 의료기관에서 308명의 환자를 대상으로 cefuroxime을 24시간 이하로 사용한 환자군과 EVD 유지 기간 내내 항생제를 유지했던 환자군에서의 감염률을 후향적으로 비교하였는데 감염률은 각각 3.8%와 4.0%로 차이가 없었다[24]. 현재까지는 감염률을 낮춘다는 증거가 없고 항생제 부작용과 내성균 획득의 위험이 증가할 것이므로 다른 신경외과적 수술과 마찬가지로 감염이 없는 상태에서 EVD 유지기간 동안 예방적 항생제를 유지하는 것은 추천되지 않는다[21]. 한 가지 예외가 뇌척수액 누출이 있는 경우이다. 뇌척수액 누출이 있는 경우는 뇌실염의 위험이 매우 커진다[25]. 특히, 두개골 기저부 골절(basal skull fracture)에 동반되어 뇌척수액 누출이 있는 경우 뇌실염 발생 위험은 20% 이상까지 증가한다는 보고가 있다[26]. 저자의 경험으로도 이 경우는 뇌실염 발생 위험이 매우 크다. 적어도 뇌척수액 누출이 해결될 때까지 예방적으로 항생제를 유지하는 것이 좋겠는데, 항생제의 종류도 뇌실염 치료 때처럼 그람양성균과 음성균을 모두 포함하는 병합요법(예, vancomycin + cefepime)이 바람직하겠다.

항생제 도포 EVD 관(antibiotic-impregnated EVD catheter)의 효능은?

현재까지 시판된 항생제 도포 EVD 관에 사용된 항생제 혹은 항생물질로는 clindamycin/rifampin, minocycline/rifampin, silver가 있다. 최근에 그동안 발표된 8개의 전향적 연구에 대한 메타분석 결과가 발표되었는데[27], 항생제가 도포되지 않은 대조군과 비교할 때 EVD 관 관련 감염률은 항생제 도포 EVD 관 사용군에서 의미 있게 낮았다(3.6% vs. 13.7%, odds ratio=0.25, 95% confidence interval=0.12–0.52, P<0.05). 특히, coagulase-negative staphylococci를 비롯한 그람 양성 세균 감염률이 유의하게 낮았다. 도포된 항생물질의 종류에 따라서는 clindamycin/rifampin과 minocycline/rifampin이 silver보다 나은 결과를 보였다. 현재까지의 근거로는 clindamycin/rifampin, minocycline/rifampin 도포 EVD 사용이 권장할 만하다. 하지만, 아직까지 이에 대한 연구가 많지 않고 대상도 한정되어 있어 일률적인 추천을 하기에는 부족하며 추가적인 연구가 필요하다. 저자가 확인한 바로는 국내에서도 clindamycin/rifampin EVD 관(BACTISEAL®)이 사용 중이다.

뇌실질 압력 측정기구(Camino®) 사용시 예방적 항생제를 사용할 것인가?

뇌실질에 삽입하는 압력 측정기구는 EVD와 비교할 때 감염 발생 위험 측면에서 살펴보면 두 가지 다른 점이 있다. 한 가지는 가운데 내강(lumen)이 없어 내강을 따라 세균이 전파될 가능성이 없고, 다른 한 가지는 기구의 끝부분이 뇌실 내에 있지 않아 뇌척수액 누출과 관련된 뇌실염의 위험이 적다는 것이다. 실제로 현재 가장 많이 사용되는 Camino®와 관련된 감염 발생률은 EVD와 비교할 때 훨씬 낮게 보고된다. 최근 터키에서 Camino®를 사용했던 631명의 환자군에서 감염률을 보고하였는데, EVD를 사용하지 않고 Camino®만을 사용했던 환자군의 감염률은 1.8%로 EVD를 같이 사용했던 군의 7.9%보다 낮았다[28]. 감염이 발생한 환자들을 분석하였을 때 감염여부와 유지기간과는 유의한 상관이 없어 저자들은 Camino®의 경우 EVD 달리 사용기간에 구애받지 않고 필요한 만큼 사용할 수 있겠다고 주장하였다. 하지만, 연구에 포함된 증례들에서의 Camino® 평균 사용기간이 6.5일밖에 되지 않아 사용기간에 영향을 받지 않는다는 주장에는 무리가 있다. 실제로 Camino®사용과 관련하여 뇌농양이 발생한 증례 보고도 있다[29]. 따라서 가능하면 일찍 제거하는 것이 바람직하겠다. 예방적 항생제는 EVD와 같이 1세대 혹은 2세대 cephalosporin을 24 시간 이내로 사용하는 것이 추천된다. 지속적인 항생제 사용의 효능에 대해서는 아직까지 연구가 없으며 추천되지 않는다.

EVD 관련 뇌실염 치료에 linezolid의 역할은?

Vancomycin은 staphylococci를 비롯한 대부분의 그람양성균에 의한 뇌실염 치료에 선택적 약제이다. Vancomycin 사용에 치료 실패인 경우, 신장 기능이 좋지 못하거나 약제 부작용으로 vancomycin을 사용하지 못하는 경우, vancomycin에 감수성이 감소 되어 있거나(최소억제농도[minimum inhibitory concentration, MIC]가≥2 μg/mL인 경우) 내성인 균에 의한 감염(예, vancomycin-resistant enteorococci)인 경우 사용가능한 약제는 linezolid, daptomycin 정도가 있다. Daptomycin은 아직까지 우리나라에서 사용 가능하지 않다. Linezolid는 단백합성을 억제하는 항생제로 bacteristatic한 항생제에 해당한다는 우려가 있지만, 중추신경계로의 투과율이 비교적 좋아서 혈중 농도의 80% 정도로 뇌척수액 농도가 높다는 장점이 있다[30-32]. 아직까지 임상 자료가 많지 않지만 보고된 증례들의 경우 비교적 효과가 나쁘지 않았다[33-37]. 앞에서 언급한 경우들과 같이 vancomycin 사용이 힘든 경우 사용을 고려할 만 하다.

Extensively drug-resistant 그람음성균에 의한 뇌실염을 어떻게 치료할 것인가?

Extensively drug-resistant (XDR) 그람 음성균 감염이 전 세계적으로 문제가 되고 있다. XDR 그람 음성균의 정의는 2가지 이하 계열의 항생제에만 감수성인 경우를 의미한다[38]. 모든 계열의 항생제에 모두 내성인 경우를 pandrug-resistant (PDR)라고 분류한다. XDR이나 PDR인 그람음성균은 Acinetobacter baumanniiPseudomonas aeruginosa인 경우가 많은데, 최근에는 장내세균 중에서도 XDR이나 PDR인 경우가 늘고 있다. XDR인 그람음성균에 의한 뇌실염의 치료 옵션은 매우 제한적이다. XDR의 경우 정맥 주사제로 사용할 수 있는 약제가 거의 없는 경우가 많다. XDR A. baumannii에 감수성인 약제는 대개 tigecycline, colistin (polymyxin E), amikacin과 같은 aminoglycoside, sulbactam 정도인데 이 약제들은 모두 뇌척수액으로의 투과율이 좋지 않기 때문이다. 뇌실 내로 직접 항생제를 투여하는 방법이 시도되고 있는데, colistin, gentamicin 정도에서 많지 않은 임상자료가 있다[39-41]. 우리나라나 외국 모두에서 식품 안전처의 허가를 받지는 못했다. 지금까지 보고된 바로는 대부분 뇌실 내로의 직접적 항생제 투여가 효과가 있었다는 보고가 많은데, 반응이 좋았던 경우들이 주로 보고되었을 가능성이 있다. 투여 용량은 정해진 바는 없으나 colistin은 10 mg, gentamicin은 5 mg 정도를 24 시간 간격으로 투여하는 것이 권장되며 투여 후 1 시간 정도 EVD를 잠가 놓는 것이 좋다. 뇌실 내로 항생제 투여 시 정맥 주사를 같이 투여하는 것이 권장된다. 뇌실 내로 직접 항생제를 투여할 경우 화학성 뇌실염(chemical ventriculitis)의 위험이 있고 신경독성의 위험도 있다.

결 론

EVD 관련 뇌실염 분야의 중요한 변화는 EVD 관련 뇌실염을 예방하기 위한 노력으로 bundle 개념이 도입되어 여러 가지 방법들이 함께 시도되고 있다는 점과 다제그람음성 세균에 의한 뇌실염이 문제가 되고 있다는 점이다. 결론부분에서는 bundle에 대해 좀 더 언급하고자 한다. Bundle 개념은 중환자실을 중심으로 중심정맥관 관련 감염이나 인공호흡기 관련 폐렴, 유치도뇨관 관련 요로감염을 대상으로 그 효과를 입증 받은 방법으로, ‘기구 관련 감염 예방을 위해 효과가 있다고 알려진 방법들을 하나의 묶음으로 모아서 의료진들이 확인하고 지키도록 하는 것’이라고 요약할 수 있겠다. 즉, 지금까지 알려진 증거들의 종합적 적용이라고 할 수 있는데, EVD 관련 감염예방을 위해서도 타당한 접근이라 생각된다. 현재까지 EVD 관련 감염을 줄이기 위한 방법으로 인정받고 있어 bundle에 포함될 만한 것들을 Table 1에 정리하였다[6,11,12,42-45]. Bundle에 포함되는 항목에 대해서는 다양한 의견이 있을 수 있다. 정기적으로 EVD를 교체할 것인가 하는 문제와 뇌실염의 조기 진단을 위해 정기적으로 뇌척수액 검사를 할 것인가 하는 문제는 결론이 나지 않았는데, 이 문제들에 대한 공고한 결론은 앞으로도 나기 힘들다. 저자의 의견은 뇌실염의 증상이나 징후가 없는 경우 뇌실염의 조기진단을 위한 뇌척수액검사는 그 위험도가 확연히 증가한다고 알려진 5일째부터 시작하고, EVD 교체는 7일 정도의 주기로 한다면 뇌척수액 검체 채취로 인한 감염 위험도를 최소화하면서 뇌실염의 조기진단도 가능할 것이다. 하지만, 이에 대해서는 아직까지 믿을 만한 증거가 부족하므로 앞으로의 연구 결과들을 참조해야겠다.
