Stroke Guidelines: From Past to Future

ABSTRACT
This article discusses the past, present and future in the treatment of ischemic stroke. Currently, many studies in ischemic stroke are ongoing and stroke guidelines in Korea are necessary in these fields as followings: a) Use of prehospital screening instrument and enhancing role of paramedicine in field treatment in Korea. b) Optimized thrombolysis and recanalization therapy using multimodal neuroimaging (CT and MRI) in Korea, where intracranial atherosclerotic stroke is prevalent. c) Guideline for prevention of deep vein t! hrombosis in Korea, where the prevalence of deep vein thrombosis in relatively rare. d) Cerebral perfusion enhancing strategies in Korea, where large and small arterial disease are prevalent. e) Guideline for hyperglycemia control at hyperacute stage of stroke. f) Fever control and application of hyperthermia as a neuroprotective agent. g) Hyperventilation and respiratory care in acute stroke. h) Best treatment of intracranial and subarachnoid hemorrhage in Korea, where hemorrhagic stroke is still prevalent.
