신경계 중환자실에서 시행한 경피적 확장 기관절개술의 안정성과 타당성

Abstract

Background

Percutaneous dilatational tracheostomy (PDT) is one of the most commonly performed procedures on critically ill patients, and many studies have shown the safety and feasibility of PDT, but there is limited data of PDT in neurocritical care units. We described our experience on PDT performed by neurointensivist.

Methods

The PDTs were performed by a neurointensivist at the bedside using the Griggs guide wire dilating forceps technique. To confirm a secure puncture site, the PDT was done under fiberoptic bronchoscopic guidance. From September 2015 to August 2017, procedural data were prospectively collected, and the patients’ demographic and clinical characteristics were retrospectively reviewed. We analyzed immediate complications of PDT as the primary outcome.

Results

The PDTs were performed for 46 patients; and the mean age was 65.9 years, 26 (56.5%) were male, and the mean acute physiology and chronic health evaluation II score was 20.5. Overall, the procedural success rate was 100%, and the mean procedural time was 19.7±9.3 minutes. Periprocedural complications occurred in 13 (28.3%) patients; with 10 having minor bleeding and three having a tracheal ring fracture. There were no serious periprocedural complications of PDT.

Conclusion

From our experience, the PDT in the neurocritical care unit was safe and feasible and was implemented without serious complications.

서 론

기관절개술은 중환자실에서 흔히 사용하는 술기이며, 장기간 기계환기가 필요하거나 기도 확보가 필요한 경우, 기도 협착이 있는 경우, 가래 등 기도 분비물이 많은 경우 시행한다[1]. 1955년 Shelden 등[2]에 의해 경피적 기관절개술이 소개된 이후 많은 발전이 있었고[3], 최근에는 수술적 기관절개술 외에 비수술적 방법에 의한 경피적 확장 기관절개술(percutaneous dilatational tracheostomy)이 중환자실에서 많이 시행된다. 경피적 확장 기관절개술은 수술장으로 이송 없이 침대 옆에서 시행 가능하기 때문에 환자 이송으로 인하여 발생할 수 있는 문제들을 예방할 수 있으며, 수술적 방법에 비하여 출혈, 감염 등 부작용의 발생을 줄여준다[4-6]. 또한 수술적 기관절개술에 비하여 쉽게 배울 수 있으며, 경험이 많지 않은 의사도 비교적 안전하게 할 수 있어 중환자실에서는 수술적 방법 대신에 많이 사용한다[7-10].
중환자실에서 수행된 경피적 확장 기관절개술의 안정성 및 효과에 대해서는 많은 연구 결과들이 있으나 신경계 중환자실에서 수행된 경피적 확장 기관절개술에 대한 연구들은 많지 않다[11,12]. 더욱이 우리나라의 신경계 중환자실에서 경피적 확장 기관절개술은 아직 보편적인 술기가 아니며 이에 대한 자료가 전무하다. 이에 저자들은 신경계 중환자실에서 시행된 경피적 확장 기관절개술의 경험에 대하여 기술하고자 한다.

대상 및 방법

1. 자료 수집 및 분석

2015년 9월부터 2017년 8월까지 2년 동안 신경계 중환자실로 입원한 환자들 중에서 경피적 확장 기관절개술을 받은 46명의 환자들이 포함되었다. 경피적 확장 기관절개술 시술 과정 및 부작용에 대한 자료들은 전향적으로 수집하였다. 시술자가 시술 일주일 뒤 기관내관(tracheostomy tube)을 처음으로 교환하면서 부작용의 발생 여부를 추가로 확인하였다. 환자의 나이, 성별 등 기본적인 정보와 진단명, 검사 결과 등 임상적인 정보는 의무기록 조사를 통해 후향적으로 수집하였다. 본 연구에서의 모든 분석은 SPSS version 21.0 (IBM Corp., Armonk, NY, USA)을 이용하였다.

2. 시술 방법

경피적 확장 기관절개술은 신경계 중환자실 전담의 혹은 신경계 중환자실 전담의의 감독하에 신경계 중환자실 전임의에 의해 수행되었다. 기관절개술 시기 및 방법은 신경계 중환자실 전담의와 해당 주치의의 논의에 의해 결정하였다. 경피적 확장 기관절개술은 기관지 내시경 감시하에서 수행되었으며 Griggs guide wire dilating forcep technique을 사용하였다.
시술 중 저산소증 발생을 예방하기 위해 시술 중 기계환기의 흡입산소농도(fraction of inspired oxygen)는 100%로 유지하였다. 시술 전 진정을 위해 미다졸람(midazolam) 3 mg을 정맥 투여하였고, 진정 상태에 따라 미다졸람 3 mg을 추가로 투여하였다. 환자가 진정 상태가 되면 어깨 아래에 베개를 넣어 어깨를 높게 하고 목을 과신전(hyperextension)시켜 기관(trachea)이 잘 노출되도록 자세를 잡았다. 시술 부위를 클로르헥시딘(chlorhexidine)으로 소독하고 수술포로 덮은 뒤 소독된 펜을 이용하여 해부학적 위치를 표시하였다. 먼저 방패연골(thyroid cartilage)과 반지연골(cricoid cartilage)의 위치를 확인하고 아래쪽으로 기관연골고리(tracheal ring)를 촉지하여 확인한 후 위치를 표시하였다. 방패연골과 복장패임(sternal notch)을 연결하는 선을 추가로 표시하여 시술 중 기관이 지나가는 방향을 확인 가능하도록 하였다(Fig. 1).
시술 직전에 펜타닐(fentanyl) 50 mcg을 투여하였고 리도카인(lidocaine)으로 국소 마취 후 첫 번째와 두 번째 기관연골고리 사이를 횡으로 1.5 cm 절개하였다. 피하조직을 박리하여 기관을 노출시킨 후 다시 기관연골고리를 촉지하여 천자할 부위를 확인하였다. 기관지경술을 통해 기관내 튜브(endotracheal tube)의 위치를 확인하고 천자할 부위 위쪽으로 기관내 튜브 위치를 조정하였다. 작은지혈집게(mosquito forceps)로 첫 번째와 두 번째 기관연골고리 사이를 누르면서 기관지경술로 기관의 중앙 부분을 확인하였고 이후 바늘로 천자하였다. 천자 후 기관 뒷벽에 바늘이 닿지 않도록 방향을 조정하면서 바늘을 기관 아래쪽으로 밀어 넣었다. 바늘집(needle sheath)만 남기고 바늘은 제거하였고 바늘집을 통하여 기관 안으로 유도철사(guide wire)를 삽입하였다. 바늘집을 제거하고 유도철사를 따라 확장기(dilator)를 삽입하여 겸자(forcep)가 들어갈 통로를 확보하였다. 확장기를 제거하고 유도철사를 따라 겸자를 삽입하였고, 겸자를 벌려 기관내관이 들어갈 수 있는 충분한 공간을 확보하였다. 겸자를 제거하고 유도철사를 따라 기관내관을 삽입하고 시술을 종료하였다(Fig. 1).

3. 용어 정의

진정 시간은 시술 준비를 위해 약물을 투여한 시점부터 시술을 종료한 시점까지로 정의하였고, 시술시간은 피부 절개를 시작한 시점부터 시술을 종료한 시점으로 정의하였다. 시술과 관련하여 발생한 출혈이 시술 당일 기관 절개 부위에 3회 이상의 드레싱(dressing)이 필요한 경우 소출혈(minor bleeding)로 정의하였고, 수혈이 필요하거나 수술적 지혈이 필요한 경우 대출혈(major bleeding)로 정의하였다. 가장자리궤양(stomal ulcer)은 기관내관 삽입 부위 주위로 피부의 국소적 결손이 있거나 함몰을 보이는 경우로 정의하였다.

결 과

2015년 9월부터 2017년 8월까지 신경계 중환자실로 입원 환자들 중에서 기관절개술은 받은 환자들은 168명이었고, 그중에서 경피적 확장 기관절개술을 받은 환자 46명(35.9%)이 분석에 포함되었다. 환자들의 평균 나이는 65.9세(±16.5)였으며 56.5% (26명)가 남성이었다. 입원 진단명을 보면 허혈뇌졸중으로 입원한 환자들이 가장 많았으며, 다음으로 뇌내출혈, 외상뇌손상, 뇌전증지속증, 중추신경계감염, 거미막밑출혈 환자들이 포함되었다. 환자들의 acute physiologic and chronic health evaluation II score의 평균값은 20.5점(±7.0)이었다. 신경과로 입원한 환자들이 36명(78.3%)이었고 나머지는 신경외과로 입원한 환자들이었다. 항혈소판제 혹은 항응고제를 복용중인 환자들 19명(41.3%)이 포함되어 있었지만 시술 당일 시행한 검사실검사에서 혈소판감소증, 응고병증을 보인 환자들은 없었으며 동맥혈가스분석에서 산소분압 및 이산화탄소분압은 정상 범위였다(Table 1).
기관절개술을 시행한 이유로는 기도 확보를 위하여 시행한 경우가 24명(52.2%)으로 가장 많았으며, 장기간 기계환기가 필요한 경우가 17명(37.0%)으로 그 다음이었다. 그 외에도 신경근육병의 진행으로 인하여 기관절개술을 받은 환자가 3명(6.5%) 포함되었다. 기관절개술 시행 전 중환자실 체류 기간의 평균은 14.0일(±8.5)이었다. 모든 환자들은 시술 전 기관내 삽관 중이었고 기관내 삽관 기간의 평균은 10.7일(±5.8)이었다. 36명의 환자들은 기계환기를 적용하고 있었으며, 기계환기 적용 기간의 중간값은 9.0일(2.0–15.0)이었다(Table 1).
2년 동안 경피적 확장 기관절개술을 시행한 환자들 46명 모두에서 시술은 성공적으로 시행되었다. 경피적 확장 기관절개술을 수행하는 데 걸린 시술시간의 평균값은 19.7분(±7.3)이었고, 진정시간의 평균값은 34.7분(±17.0)이었다. 경피적 확장 기관절개술과 관련된 부작용은 13명(28.3%)에서 발생하였다. 시술과 관련된 출혈이 10명(21.7%)에서 발생하여 가장 흔한 합병증이었으며, 시술 도중 기관지경술 검사에서 기관연결고리 골절이 확인된 환자가 3명(6.5%)이었다. 출혈은 모든 환자에서 24시간 이내에 지혈이 되었고 수혈이나 수술적 지혈이 필요한 경우는 없었다. 시술자가 시술 일주일 뒤 처음으로 기관 내관을 교환하면서 부작용 발생 여부를 확인하였을 때 가장 자리궤양이 5명(10.9%)에서 확인되었다(Table 2).
항혈전제 복용에 의한 출혈 위험성을 확인하기 위하여 항혈전제 복용 유무에 따라 두 군으로 나누어 추가적인 분석을 진행하였다. 항혈전제를 복용 중인 환자 중에서 항혈소판제를 복용 중인 환자가 17명이었고, 항응고제를 복용 중인 환자가 2명이었다. 모든 환자들은 항혈전제 중단 없이 시술을 받았으며, 시술 당일 혈액응고검사에서 이상이 발견된 환자는 없었다. 항혈전제 복용 유무에 따른 시술 시간의 차이는 없었으며, 항혈전제를 복용 중인 환자에서 출혈 부작용이 증가하는 경향을 보였으나 통계적으로 유의미한 차이는 없었다(Table 3).

고 찰

본 연구는 2년 동안 신경계 중환자실에서 시행된 모든 경피적 확장 기관절개술을 분석하여 시술의 용이성과 안정성을 확인하고자 하였다. 경피적 확장 기관절개술을 받은 46명의 환자 모두에서 시술은 성공적으로 수행되었고, 시술 시간도 19.7분으로 짧은 시간에 시술이 가능하였다. 출혈이 10명(21.7%)에게서 발생하여 가장 흔한 부작용이었으나 수혈이나 수술적 지혈 등 추가적인 치료가 필요한 경우는 없었다. 7일째 추적관찰하였을 때에도 가장자리궤양이 5명(10.9%)에서 발생한 것 외에 다른 부작용은 없었다. 이를 통해 신경계 중환자실에서 경피적 확장 기관절개술이 심각한 부작용 없이 안전하게 시행될 수 있다는 것을 확인할 수 있었다.
경피적 확장 기관절개술의 장점은 이미 여러 전향적 연구 및 메타분석에서 입증되었다[4-6,13]. 경피적 확장 기관절개술은 환자를 수술장으로 이송하지 않고 침대 옆에서 술기를 할 수 있어 이송에 따른 위험을 줄여준다[14]. 또한 수술적 방법에 비해 작은 피부 절개로도 가능하며 수술에 따른 주위 조직 손상이 적게 발생하여 출혈, 감염 등 부작용의 발생을 감소시킨다[4-6]. 기관절개술을 받은 환자들을 장기간 추적관찰한 연구에서도 수술적 기관절개술에 비하여 감염과 흉터의 발생을 줄여주었다[6]. 본 연구에서는 대조군이 없어 수술적 기관절개술과 직접적인 비교는 할 수 없었지만 다른 연구에서 보고된 경피적 확장 기관절개술의 전체 합병증 발생률(12–25%)과 큰 차이를 보이지 않았다[4,15]. 전체 합병증에서 출혈 합병증이 다른 연구(4–12%)에 비하여 높았으며 이는 항혈전제를 복용 중인 환자들이 많이 포함된 것이 영향을 미쳤을 것으로 생각된다[4]. 또한 시술 중 전기지짐기(electrocautery)를 사용하지 않아 시술 중 충분한 지혈을 할 수 없었던 것도 시술 직후 출혈의 발생을 증가시켰을 것으로 생각된다. 하지만 모든 출혈들이 24시간 이내에 지혈이 되었고 수혈이나 수술적 지혈이 필요한 경우는 없었다. 감염의 발생은 다른 연구(4–10%)에 비하여 적었는데 이는 대부분의 환자들이 다른 원인으로 항생제를 사용하고 있었고 전기지짐기를 사용하지 않아 조직 손상이 적었던 점이 감염에는 유리하게 작용하였을 것으로 생각된다[4].
저자들은 경피적 확장 기관절개술로 인한 부작용을 감소시키기 위하여 여러 감시 장비들을 이용하였다. 시술 전 초음파를 이용하여 시술 부위에 동맥이나 정맥이 위치하는지를 확인하였다[16]. 시술 도중에는 기관지경술을 통하여 시술 과정을 전체적으로 감시하였고 바늘 천자 위치가 적절한지, 바늘이나 겸자가 기관 뒤쪽 벽에 닿아 추가적인 기관 손상이 발생하는지, 유도철사와 기관내관의 삽입 방향이 적절한지 등을 확인하면서 시술을 진행하였다[17]. 또한 기관지경술 감시 중 발생할 수 있는 저산소증, 고탄산혈증 등을 예방하기 위하여 시술 전 흡입산소농도와 일회호흡량(tidal volume)을 증가시킨 상태에서 시술을 진행하였다.
경피적 확장 기관절개술은 수술경험이 없는 신경과 의사들도 비교적 간단히 수행할 수 있는 술기이다[11]. 본 연구에서도 실제 시술에 걸리는 시간은 20분 내외였으며 이는 다른 연구에서 보고된 시술시간과 큰 차이가 없었다[7,8,18]. 내과계 중환자실에서 시행된 이전 연구 결과를 보면 수술경험이 없는 내과의사들도 심각한 부작용의 발생 없이 경피적 확장 기관절개술을 수행하였으며, 이비인후과 의사에 의하여 시행된 수술적 기관절개술에 비하여 오히려 시술시간이 유의미하게 감소하였다[7]. 최근에는 상용화된 장비를 사용하여 비교적 간단하게 술기를 시행할 수 있으며 술기에 익숙해지는 기간도 짧다. 중환자실에서 수련 중인 전임의와 시술경험이 많은 중환자실 전담의에 의하여 시행된 경피적 확장 기관절개술을 비교한 연구 결과를 보면 시술시간과 부작용의 발생 모두 유의한 차이를 보이지 않았다[8]. 또한 전공의들도 쉽게 경피적 확장 기관절개술을 배울 수 있으며 5회 이상 경험이 축적된 뒤에는 합병증의 발생이 유의하게 감소하였다[19].
신경계 중환자실 환자들은 호흡기계 합병증이 흔히 동반되며 장기간 기계환기가 필요하거나 의식 저하로 인하여 기도 유지가 어려운 경우가 많다[20-22]. 이로 인하여 기관절개술이 필요한 환자들이 많으며 최근에는 기관절개술을 조기에 시행하는 것이 중환자실 입원 기간과 기계환기 적용 기간을 감소시켜 준다는 연구 결과도 있다[23-25]. 하지만 수술적 기관절개술의 경우 수술을 담당하는 이비인후과 의사의 수술 일정과 수술장 사정 등으로 인하여 연기되는 경우가 많다. 또한 부분 마취로 기관절개술을 시행하기 때문에 수술장에서 적절한 감시가 이루어지지 않는 경우도 있다. 따라서 신경계 중환자실에서 경피적 확장 기관절개술의 적용은 신경계 중환자실을 담당하는 의사들이 기관절개술 시행 시점을 결정할 수 있고, 시술 중 발생할 수 있는 합병증 등에 대하여 적절한 감시가 가능하다는 장점이 있다.
이번 연구는 몇 가지 한계점을 가지고 있다. 첫째, 본 연구는 단일기관에서 경피적 확장 기관절개술을 받은 환자들을 대상으로 한 후향적 연구이다. 대조군 없이 시술을 받은 환자들의 결과를 기술한 사례 연구로 선택바이어스(selection bias)가 연구 결과에 영향을 미칠 수 있다. 둘째, 경피적 확장 기관절개술을 받은 환자들의 시술 직후와 7일째의 부작용을 확인하였기 때문에 단기 부작용은 평가할 수 있었으나 시술로 인한 장기 부작용은 평가할 수 없었다. 셋째, 시술자들이 직접 시술로 인한 부작용을 평가하였기 때문에 이로 인한 바이어스가 발생하였을 가능성이 있다.

결 론

신경계 중환자실에서 신경계 중환자실 전담의에 의하여 시행된 경피적 확장 기관절개술은 모든 환자에서 성공적으로 시행되었으며 심각한 초기 부작용은 없었다. 경피적 확장 기관절개술은 신경계 중환자실에서 간단하고 안전하게 수행 가능한 술기이다.
